Multidisciplinary Treatment For Severe Periodontitis:10-Year Follow-Up.
We used the combined periodontal-orthodontic-restorative treatment to prevent periodontitis progress, and improved the occlusal function . The patient had good compliance, better plaque control, regular periodontal maintenance, and achieved a stable treatment effect after 10-year follow-up. Ms.Xing, 30y, her mother and sister both had periodontitis. The anterior teeth were crowded, multiple teeth loose for 2 years, BOP 40%, PI 2, marginal tissue ression Miller IV. OPG:alveolar bone horizontal resorption II°; 31,41 to 1/3 of the root . Diagnose: Chronic periodontitis (stage III, grade C); Malocclusion. Treatment plan: periodontal-orthodontic-restorative combination therapy. In this case, the patient had both primary and secondary occlusal trauma. For patients with good compliance and control of periodontitis, orthodontic treatment can reduce the occlusal trauma, align the dentition for plaque control, and provide space for restorative treatment. Subsequently, the minimally invasive periodontal splint type fixed bridge was adopted to achieve the purpose of stable residual tooth occlusion. For severe periodontitis, satisfactory results require two points: First, thorough debridement and continuous plaque control. Both regular professional maintenance and good self-plaque-control are needed. Second, the restoration of occlusal function and bite force rebalancing. In this case, the personalized periodontal-orthodontic-restorative combination treatment was used, which stabilized the overall occlusion. In short, a complete diagnosis and treatment of severe periodontitis cases requires a comprehensive treatment plan, standardized treatment procedures, and regular and long-term review and maintenance.
