Personalized Clasp-Free Implant-Supported Removable Partial Denture
This report focuses on elderly patients over 70 years old with distal extension edentulism, previous removable partial dentures (RPDs) experience, and stage III periodontitis. We present a personalized clasp-free, implant-supported removable partial dentures (ISRPDs) design, offering clinical insights for rehabilitating these complex cases. This report presents three clinical cases of elderly patients (>70 years) with Kennedy Class I dental defect, stage III periodontitis and previous experience with conventional RPDs. The prosthetic design was determined through comprehensive evaluation of: patient preferences (tooth preservation and functional/aesthetic improvement), treatment acceptance (limited implant placement and removable prosthesis), oral/systemic health status, implant lifecycle considerations, prosthetic functional requirements. The treatment involved: personalized clasp-free ISRPD design, mesial implant placement adjacent to edentulous spaces, customized attachment systems. This ISRPD design reduced biomechanical strain on residual teeth and implants while improving prosthesis performance. This design balanced patient preferences (tooth preservation, affordability, and mini-invasive treatment) with clinical constraints (bone quality, systemic health). Strategic implant placement and attachment selection mitigated challenges of distal extension cases, offering predictable retention and tissue preservation. The report highlight the importance of personalized planning, considering implant lifecycle and functional demands, to achieve sustainable outcomes in elderly patients with complex dental and periodontal conditions. In summary, for elderly patients with Kennedy Class I dental defect accompanied by periodontitis stage III, personalized design of clasp-free ISRPDs were applied. By strategically selecting implant sites considering the patients' wishes and clinical conditions, this approach successfully improved denture retention, stability, comfort, and esthetic satisfaction.
