Full-Arch Implant Restoration Involving Transdental Implant Placement: Case Report

Transdental implant placement—where implants are inserted through impacted natural teeth—is an emerging minimally invasive technique, particularly suitable for elderly patients, those with insufficient bone volume, or patients unwilling to undergo conventional tooth extraction. Panoramic radiograph revealed: Tooth #13: Horizontally impacted, extending from distal #12 to mesial #16, with buccal protrusion through cortical bone. Residual roots #22 and #23 with periapical radiolucency. Right maxillary sinus: Pneumatized, with 2–3 mm of usable bone at #16–17 sites. Left maxillary sinus: Presence of a bony septum; ∼10 mm of usable bone at #26–27. Diagnosis:Edentulism (maxilla and mandible);Residual roots and bone defects; Impacted canine (maxillary right);Failing prostheses in the maxilla Treatment Plan: improve treatment efficiency. Treatment Procedure: Maxillary Arch: Simultaneous immediate implant placement and selective extraction. During osteotomy, dentin tissue of the impacted canine was encountered, confirming implant penetration through the retained tooth. Prosthetic Phase: Definitive prostheses delivered following standard healing protocols. Follow-Up: Occlusion remained stable at 1, 3, and 6 months. Chewing function restored. No signs of implant mobility or peri-implant complications. In this case, the right maxillary canine was severely impacted across sites #13–15, with minimal available bone due to sinus pneumatization. Given the patient’s advanced age (88 years) and general condition, the team proposed a transdental implant plan after full disclosure of existing literature, associated risks (e.g., possible implant failure), and alternative treatments (e.g., implant-supported overdentures). The patient gave informed consent. Transdental Implant Placement need more considered.
