"All In One Time" Crown Lengthening Surgery, A Case Series.

Crown lengthening surgery is an effective procedure to improve aesthetics for patients with "gummy smile". Traditional crown lengthening surgeries often involve complex steps and require a long waiting period from initial consultation to the completion. The AOT (All in One Time) crown lengthening approach, however, can finish the procedure in one session at the initial consultation, dispensing with the need for surgical guides. Fifteen patients who required aesthetic crown lengthening were included in this case series. Patients' intraoral and extraoral smile photographs and pre-operative crown length measurements are imported into the DSD software for esthetic design. The software will calculate the amount of gingival that should be removed and display patient the monitored post-operative smile images. Subsequently, minimally invasive crown lengthening surgery with a novel sutures technique is performed based on the design. The meaning of AOT technique is that it not only can shorten the time of therapy, but also can acquire a more esthetic outcome in one short session by using DSD, minimally invasive surgery and novel suture technique. In this report, the average of VAS points is 8.07±0.7 immediate after surgery and remain steady. And the crown lengths in all 15 patients have prominence change compared to the preoperative crown length (p<0.05). As a result, this approach reduces the number of visits and shortens the treatment period, achieving aesthetic outcome in one session, with immediate aesthetic improvement.
