Early Sequential Treatment Of Impacted Maxillary Incisor

Comprehensive Management of an Impacted Maxillary Incisor: Early Intervention, Frenectomy, and Caries Control A 7-year-old girl presented with delayed eruption of tooth 11 (labially impacted) and caries. Treatment (April 2021 - October 2024, 42 months included: 1. Phase I (April - November 2021):Laser-assisted fenestration/traction and frenectomy achieved 50% eruption of tooth 11. 2. Phase II (May - August 2021):Caries management (restorations, extractions, space maintenance). 3. Phase III April 2022 - October 2024): MRC appliance therapy with myofunctional training corrected tongue thrust and breathing. Outcome (August 2024 Follow-up): - Tooth 11 fully erupted (15° rotation, root stage VIII, apical curvature). - Permanent dentition established: Class I incisor/molar relationships, midline alignment. - MRC therapy successfully discontinued. 1. Etiological Analysis: The patient had a history of trauma to the primary incisors, which is the most common cause of impacted maxillary central incisors. Clinical implication: The impaction of tooth 11 in this case was likely secondary to prior primary tooth trauma. 2. Surgical Advantages of Laser Technique: -Laser-assisted fenestration demonstrated three significant benefits: a) Reduced operative time b) Minimized postoperative inflammation c) Improved bracket survival rate Early intervention for impacted maxillary central incisors at the appropriate time can effectively improve the success rate of traction. Combined with laser-assisted traction, it can reduce pain and discomfort during the treatment process, providing a more minimally invasive and comfortable therapeutic approach for such conditions.
