A Rare Case Report Of Tc Invading The Oral Cavity

We report 1 rare case of Trichilemmal Carcinoma(TC) involving the oral commissure and buccal region. A 58-year-old male presented with a firm, soybean-sized mass at the left oral commissure demonstrating rapid recent growth. The lesion invaded the left angular muscle and surrounding tissues, exhibiting a hard texture, poor mobility, clear boundaries, and limited opening. The mucosa in the oral corresponding to the tumor is hard and like a thickening of the oral fibrosis. In this case, an enlarged resection of the left orofacial mass and adjacent tissue flap reconstruction was performed under general anesthesia. A complete and extended resection of the tumor was performed approximately 1 cm outside the lesion, with the deep aspect reaching the mucosal layer. Postoperative recovery was uneventful. Histopathological examination confirmed trichilemmal carcinoma invaded the angular muscle up to the oral mucosa. At present, no literature mentions cases where TC invades the oral cavity. TC is a rare malignant neoplasm of cutaneous appendage originating from the epithelium of the outer root sheath of the hair follicle. The lesion is usually isolated and presents as an exophytic, polypoid, or nodular mass, often occurring on skin exposed to sunlight. TC is usually non-invasive, with only localized damage, and can be successfully treated by extensive excision. This represents the first clinical case demonstrating oral cavity invasion, and this case providing some reference for the diagnosis and treatment of TC, and offering insights for the diagnosis of oral and maxillofacial tumors.
