Nd:YAG Laser (1064 Nm) Excision Of Vascular Epulis

This prospective clinical study evaluated the Nd:YAG laser's coagulation properties for excising vascular gingival epulis lesions, focusing on hemostatic efficacy and wound healing characteristics. Materials and Methods: Twenty-eight patients with vascular epulis confirmed by Doppler ultrasound underwent excision using an Nd:YAG laser (1064 nm, 3W pulsed mode, 300μm fiber) with non-contact technique and intermittent air cooling. Outcomes included intraoperative bleeding score (0-3 scale), coagulation depth by histology, postoperative edema measurements, healing timeline, and patient satisfaction surveys. Results: The Nd:YAG laser achieved complete hemostasis in 92.9% of cases with mean coagulation depth of 0.4±0.1mm. Postoperative edema was reduced by 78% compared to scalpel controls, with complete healing in 14-21 days. Patient satisfaction reached 96%. The Nd:YAG laser's 1064nm wavelength enables superior hemostasis through hemoglobin absorption, ideal for vascular lesions. While healing occurs via secondary intention (14-21 days), this is offset by significantly reduced bleeding risks, making it particularly valuable for high-risk patients. The technique eliminates sutures while maintaining excellent tissue precision with minimal thermal damage (0.4mm depth). High patient satisfaction reflects these clinical advantages. The Nd:YAG laser provides exceptional hemostasis for vascular epulis with acceptable healing timelines. Its deep coagulation capability is particularly suitable for hypervascular lesions while requiring precise parameter control.
