"Split-Mouth Rct: Cgf In Alveolar Preservation Postextraction"
This split-mouth randomized controlled trial evaluated autologous concentrated growth factor (CGF) for alveolar preservation following mandibular third molar extraction. Thirty patients requiring bilateral impacted mandibular third molar extraction received CGF fibrin matrix in one socket (control: natural healing). CBCT scans at baseline and 3 months assessed bone parameters (height/width/density/socket volume), with secondary outcomes including wound healing, facial swelling (days 1-7), and second molar periodontal pocket depth. Nonparametric tests assessed outcomes (P<0.005). CGF sites showed superior bone preservation: buccal height (32.9±3.5 vs. 29.4±4.3 mm), lingual height (26.4±3.5 vs. 23.1±4.0 mm), and width (21.07±1.55 vs. 19.53±1.90 mm). Bone density increased markedly in CGF sites (coronal: 200±127.3 vs. -84.1±121.3 HU; apical: 406.5±103 vs. 64.2±158.6 HU). Wound healing scores improved significantly (P=0.000), with reduced swelling on day 1 (1.01±0.57 vs. 1.55±0.56) and day 3 (1.42±0.8 vs. 2.63±1.2). Periodontal pocket depth reduction was greater in CGF group (P=0.000). CGF application significantly reduced bone resorption, enhanced bone formation, and decreased postoperative complications, demonstrating clinical efficacy in alveolar preservation.
