Pulpotomy With All-Ceramic Prefabricated Crown Restoration For Primary Molar
A 7-year-old child with a chief complaint of food impaction and pain in the lower left posterior tooth for over 3 months. Diagnosis of pulpitis in the primary molar tooth was made. Vital pulp preservation combined with preformed all-ceramic crown restoration was provided and good clinical efficacy was achieved. White filling and secondary caries were observed on the 74th distant and middle adjacent surface, with discomfort during probing, pain during cold diagnosis, percussion (-), no looseness, and no abnormalities in the gums. X-ray shows: Low density imaging of the 74th distal adjacent surface reaches the medullary cavity, with the presence of permanent tooth germ and continuous hard bone plate on the root side. No obvious abnormalities are observed in the bifurcation area of the root, and there is no significant resorption of the root. At the 12-month follow-up after treatment: The restored crown showed tight marginal adaptation. Radiographic examination revealed no apical abnormalities in the affected tooth. Rubber dam isolation ensured the success of vital pulp preservation, as maintaining healthy pulp promoted physiological root resorption of primary teeth and normal exfoliation for permanent tooth eruption. The preformed all-ceramic crown provided coronal seal, restoring masticatory function. With its superior aesthetics and stable performance, this technique is expected to gain broader adoption as materials science advances and patient/family aesthetic expectations for primary tooth restorations increase.
