Autologous Fat Grafting With Botulinum Toxin For Facial Deformities
This study evaluates the efficacy of autologous fat grafting combined with botulinum toxin (BTX) injection in improving fat survival and restoring facial contours, especially in cases with soft tissue deficiencies, scars, and subcutaneous adhesions. Forty-seven patients were treated and categorized into three groups: (1) facial contour irregularities (e.g., post-orthognathic surgery asymmetry), (2) contour deformities with scars (e.g., cleft lip repair), and (3) severe subcutaneous adhesions treated with multilevel subcision and high-volume fat grafting. Fat was harvested via liposuction, processed, and injected with BTX to reduce muscle-induced shear forces. Postoperative three-dimensional imaging showed significant improvements in facial symmetry, contour, and volume retention. BTX-assisted fat grafting resulted in higher fat survival rates and improved scar texture. In cases with severe adhesions, this combined approach enhanced soft tissue augmentation and mobility. No severe complications were observed. Overall, BTX-enhanced autologous fat grafting proved to be a safe, effective treatment for facial soft tissue deficiencies, especially for post-surgical deformities, scars, and adhesions, offering a promising strategy for facial reconstruction.
