Correlation Between Periapical Lesions And Retrograde Peri-Implantitis
Retrograde peri-implantitis(RPI) might arise from apical periodontitis. This study aimed to verify the relationship between RPI and apical periodontitis(AP), while investigating if the distance between implants and AP affected the incidence of RPI. 1047 patients with 1819 implants were included in the 10-year retrospective study. 56 implants were in proximity to AP-affected teeth. Firth's logistic regression was used to access the correlation between adjacent AP and RPI. The distance between implants and AP lesion was measured by post-surgical cone beam computed tomography, categorized as either more or less than 3 mm. RPI occurrences were ascertained during follow-up by radiogragh and clinical examination. Risk factors for RPI development were also analyzed through Firth's logistic regression. The manifestation and treatment of RPI were also listed and discussed. The incidence of RPI was 10.71% when AP was present in adjacent teeth (OR: 26.51, P<0.01*). When the proximity of the implant to AP was less than 3mm, it was found to be significantly associated with the occurrence of RPI (OR=6.48, P=0.03). 5 out of 6 RPI cases related to AP received relevant treatment and showed a 83% rate survival rate. Apical periodontitis emerges as a significant risk factor contributing to the development of RPI. This study also indicated that it should be at least 3mm to adjacent AP when placing implants.
