Application Of Implant-Supported Provisional Healing Abutments In Anterior Aesthetics

This case utilizes implant-supported transitional healing abutments for gingival emergence profiles and aesthetic restoration of maxillary anterior teeth in habitual mandibular protrusion. A young male with maxillary anterior tooth loss for 3-year due to trauma. Clinical and radiographic evaluation revealed missing teeth 11,21, along with tooth 22 exhibiting a subcrestal root fracture located 1 mm apical to the alveolar ridge. Treatment plan: Extraction of tooth 22 followed by prosthetic rehabilitation with an implant-supported fixed dental prosthesis replacing teeth 11, 21 and 22.At the 6-month postoperative interval, the incisal third of the implant-supported provisional prosthesis was resected, followed by personalized healing abutments shaping peri-implant soft tissue . After 3 months of shaping, the complete zirconia fixed bridge restoration was completed. One-year follow-up shows stable implants, stable gingival emergence profiles, and no abnormalities in occlusion. Shaping gingival emergence profile with implant-supported provisional restoration in the anterior tooth missing area enhances the final aesthetic outcome (Slagter et al 2021). Due to the patient's long-term maxillary anterior tooth loss and habitual mandibular protrusion, the incisal third of the provisional restoration was trimmed to prevent fracture from excessive non-axial forces. Personalized healing abutments were used to shape the gingival emergence profiles, achieving an optimal aesthetic result. The implant-supported transitional healing abutment provides a new strategy for gingival emergence profiles shaping in patients with anterior tooth missing, where mandibular protrusion or tight occlusion may damage the provisional restoration.
