Functional And Aesthetic Maxillary Reconstruction With Digital Zygomatic Obturator Prostheses

To evaluate the efficacy of a fully digital workflow integrating zygomatic implant-supported obturator prostheses for immediate and secondary rehabilitation of maxillary defects post-tumor resection. A retrospective analysis was performed on 13 patients who underwent bilateral maxillectomy between December 2017 and March 2025. Surgical planning utilized virtual simulations and 3D-guided surgery with patient-specific templates to accurately define tumor boundaries and ensure precise placement of zygomatic implants. Post-tumor excision, immediate loading of prefabricated zygomatic implant-supported obturator prostheses was achieved through a digital workflow. For secondary reconstructions, AI-assisted regeneration of upper jaw profiles guided the design of aesthetic and functional obturator prostheses. Outcomes were evaluated via clinical assessments and finite element analysis (FEA) simulating implant fatigue under functional loads. Of the 13 patients, 3 received immediate loading of digitally planned, prefabricated zygomatic implant-supported obturator prostheses, while the remaining 10 underwent secondary rehabilitation. The immediate loading group demonstrated significant enhancements in esthetic and functional outcomes, with high patient satisfaction, compared to conventional methods. FEA demonstrated that zygomatic implants could endure simulated functional loads of up to 5 million cycles at 300N without failure, confirming their long-term reliability. Integrating a fully digital workflow—encompassing virtual planning, 3D-guided surgery, and AI-assisted prosthesis design—substantially enhances aesthetic and functional rehabilitation in patients with maxillary defects post-tumor resection. Zygomatic implant-supported obturator prostheses provide a dependable solution for complex maxillary reconstructions, supported by both clinical outcomes and computational analyses.
