Implant-Oriented Mandibular Reconstruction With Digital Technology-Assisted Iliac Flap

The Vascularized iliac crest flap plays an irreplaceable role in the recovery of mandibular defects caused by tumors, infections, or trauma. However, the complicated shape of personalized iliac crest flap and the position error after implantation lead to the difficulty of late repair. The perfect combination of CTA and digital technology design assistance provides a new strategy to solve this problem. In this study, 20 patients with mandibular defects who underwent iliac crest flap reconstruction were retrospectively analyzed. It was found that the height, width and length of the iliac crest flap designed by CTA were well matched and healed with the mandible after surgery in all patients. Digital technology assisted surgical process accurate positioning, osteotomy and implant placement. The personalized iliac crest flap maintained the vertical height of the mandible, the relative space of the implant and the facial contour, while the fixed denture supported by the implant restored good occlusal relationship. In short, the surgical plan which it combined with digital technology, personalized iliac crest flap by CTA, 3D printing intraoperative guide and implant denture restoration can perfectly restore the patient's occlusal function and facial appearance, and is an ideal plan for patients with mandibular defects.
