Combined Silicone Aligners And Mandibular Appliance For Malocclusion
Early intervention for Angle II Class 1 malocclusion offers various functional appliances. This report presents the clinical outcomes of using a combination of silicone aligners and an improved mandibular repositioning appliance. A 7-year-old child presented with protrusive upper incisors and a retrusive chin, diagnosed with Angle II Class 1 skeletal malocclusion. After wearing silicone aligners nightly for 5 months, the child switched to an improved mandibular repositioning appliance for another 5 months. Subsequently, a new silicone aligner was used nightly for 4 months, with regular follow-up every 3 months. At the age of 10, the patient showed significant improvements in skeletal and soft tissue facial morphology, achieving normal overbite, overjet, and a near-neutral bilateral molar relationship. The lower incisors were no longer labially inclined, and simple phase II treatment with permanent teeth was anticipated. Early orthodontic intervention is crucial for correcting malocclusion. Traditional appliances alone may have limitations in terms of effectiveness, comfort, and patient compliance. The combination of silicone aligners and the improved mandibular repositioning appliance is small, adaptable, convenient for nighttime wear, and demonstrates good patient compliance. This case demonstrates that for children with Angle II Class 1 malocclusion, intervention using silicone aligners in conjunction with the improved mandibular repositioning appliance can guide mandibular repositioning during nighttime occlusion. This helps reposition the condyles within the joint socket, promoting mandibular growth and facilitating adaptive remodeling of the temporomandibular joint, leading to normal physiological function of the teeth and jawbones and achieving optimal treatment outcomes.
