Autogenous Tooth Transplantation Of The Maxillary Third Molar

This is a case of autogenous tooth transplantation. We extracted the maxillary right second molar which could not be retained due to severe caries, and transplanted the adjacent maxillary right wisdom tooth. Compared with dental implantation technology, this approach preserves the structure of the natural tooth and enables the non-functional wisdom tooth to serve a useful purpose. The patient's chief complaint was that there had been a cavity in the right maxillary posterior tooth. The examination revealed that patient's maxillary right second molar was severely decayed and non-retainable. The adjacent maxillary right third molar, though carious, had sufficient remaining dental tissue. Through preoperative CBCT, we reconstructed its morphology and matched with the tooth to be extracted. A 3D-printed model was used for socket fitting. All the outside body time limited to 20 minutes. After transplantation, The carious was filled under the rubber dam, creating a good contact with adjacent teeth.During the subsequent follow-up, X-ray examination showed that the tooth achieved excellent periodontal ligament healing. First and second molars often go missing, and third molars are ideal donors due to similar shape and size. 3D - printed models aid socket prep, cut tooth -out-of-body time. Elastic fixation is better, making transplantation more efficient and predictable. Autogenous tooth transplantation is a feasible and important method for preserving natural teeth. A thorough pre-operative analysis and strict operation procedures are crucial factors in improving the success rate of transplantation.
