Clinical Feasibility Of Transcrestal Sinus Floor Elevation In Rbh <4Mm.
Severe maxillary posterior alveolar atrophy (residual bone height [RBH] <4 mm) has traditionally been considered a contraindication for transcrestal sinus floor elevation (TSFE), limited evidence exists on the feasibility of TSFE in such extreme bone deficiency, particularly when combined with concentrated growth factor (CGF) and allogeneic or xenogeneic bone graft. This study aimed to evaluate the clinical feasibility of TSFE protocol with CGF and allogeneic or xenogeneic bone graft in patients with RBH <4 mm. A retrospective case series included 9 patients (11 implants) with RBH <4 mm confirmed by CBCT. All cases underwent TSFE with CGF and allogeneic or xenogeneic bone graft . Outcomes included 6-month implant survival rates, endosinus bone gain after surgery and after 6 monts (CBCT), and complications. Mean RBH was 2.6±0.9 mm. Endosiuns bone gain after surgery reached 8.3±2.0 mm (range: 4.47–11.03 mm) and 6.3 ±2.4 after 6 months. At 6 months, implant survival was 90.9% (10/11 implants). And there is no perforation occoured. TSFE with CGF and allogeneic or xenogeneic bone graft demonstrates promising short-term outcomes in RBH<4mm cases, achieving stable bone augmentation while minimizing perforation risks. This minimally invasive, cost-effective protocol challenges traditional contraindications, offering an alternative to complex surgeries. Limitations include small sample size and short follow-up. Rigorous case selection and technical precision are critical for success.
