Using Periodontal Plastic Procedures To Treat Nonecarious Cervical Lesions
Tunnel technique or coronally advanced flap(CAF) combined with connective tissue graft(CTG) and filling technique were used to treat one patient’s bilateral maxillary Nonecarious Cervical Lesions(NCCL), respectively. The patient’s gingival recessions with cervical lesions were diagnosed as NCCL type 3 according to “Mocogingival Esthetic Surgery” written by Giovanni Zucchelli, and it could be achieved by periodontal and filling techniques. The right maxillary canine and the first premolar have about 2-3mm gingival recessions, and the left counterparts are about 3-4mm, all of which have 1mm proximal attachment loss and 3mm attached gingiva. For the right teeth, tunnel technique combined with CTG were adopted, and for the left side, CAF combined with CTG. Then the hard tissue lesions were filled with resin material. After 4 years of observation, the outcome is still satisfactory. The NCCL has 5 types and different types has its own treatment procedure. In this case, mocogingival esthetic surgery and filling technique were utilized to achieve root coverage and hard tissue repair. As for the surgical part, small surgical trauma and coronal advanced range are the crucial concerns. That means the tunnel technique and CAF were used respectively. Although there were enough attached gingiva, CTG was still selected for the purpose of tissue stability. Then filling technique was used to cover the cervical lesion, which cannot be cured by mocogingival esthetic surgery. This periodontal plastic procedures is a effective path to treat NCCL.
