A Study Of Inflammatory Pulp Treatment For The Second Premolar
Root fracture and ceased root length development were more likely to happen in the treatment of apical periodontitis in young permanent teeth with traditional apexification. This case report presents a young permanent tooth with apical periodontitis caused by the fracture of a deformed central cusp in an 8-year-old girl. The patient presented with persistent pain in the right lower posterior tooth for two days. Clinical examination revealed slight redness and swelling in the right submandibular area, and radiographic examination showed a periapical radiolucency with widened periodontal ligament space. CBCT imaging showed that the tooth was in Nolla8 of root development. The treatment plan involved using only chemical debridement to preserve as much of the activepulp tissue at the root apex as possible, controlling the inflammation, and then covering it with the bioceramic material iRoot BP for inflammatory pulp preservation treatment. Follow-up examinations at 5 months, 1 year, and 2 years postoperatively demonstrated significant periapical healing and continued root development. The preservation of inflamed pulp improved clinical symptoms and promoted root development and apical closure, indicating that inflamed pulp tissue can regain the ability to proliferate and differentiate after inflammation is controlled, thereby restoring pulp function and enabling continued root development.
