Implant-Sandwich Bone Plate Translation Technique For Horizontal Bone Augmentation
This case introduced a novel “implant-sandwich bone plate translation” technique for horizontal bone augmentation with simultaneous implant placement. The patient was a 56-year-old woman with posterior mandibular teeth missing for one year. Oral examination showed the absence of teeth 35-37(FDI notation), CBCT revealed severe horizontal bone resorption with only 3-4mm of bone width. The implant-sandwich bone plate translation technique was performed. The buccal bone plate was split using Piezosurgery and completely separated. Then, two implants (BLT, Institute Straumann AG, Basel, Switzerland, 4.1*8mm for tooth 37, 4.1*10 mm for tooth 36) were inserted, then the buccal bone plate was repositioned using a microscrew. Residual gaps were filled with deproteinized bovine bone materials (Bio-Oss, Geistlich, Switzerland) and covered with a resorbable collagen membrane (Bio-Gide, Geistlich, Switzerland). After 9 months, the bone widths at site 36 and 37 were 8.78mm and 10.18mm respectively. The final restoration was finished. The peri-implant hard and soft tissue remained stable during the 2-year follow-up. Horizontal bone defects pose significant challenges for implant placement. Traditional techniques like GBR, titanium mesh, onlay bone grafting, and ridge splitting often lead to extended treatment duration or donor site morbidity. The novel implant-sandwich bone plate translation technique doesn't need a donor site and allows for simultaneous implant placement. This case presents a novel technique for severe horizontal bone defect. It is less invasive, more time-efficient, and can yield satisfactory outcomes.
