Immediate Implant Rehabilitation In A Patient With Thalassemia.

This clinical case report details the successful management of a 30-year-old female patient with hereditary thalassemia and severe periodontitis. After a comprehensive evaluation of the patient's systemic condition and jawbone quality, immediate implant rehabilitation of both the maxilla and mandible was performed, resulting in excellent restorative outcomes and occlusal function. The patient presented with generalized tooth mobility for over a decade, a protruding anterior maxilla, Angle Class II malocclusion, multiple missing teeth in both jaws, and remaining teeth with mobility of grade II-III except for teeth 34 and 44. Cone-beam computed tomography (CBCT) indicated Lekholm & Zarb Class III bone quality, and blood tests revealed mild thalassemia. The treatment plan involved staged extraction of the loose teeth and immediate implant placement with 7 Dentium SuperLine® implants in the maxilla and 6 implants in the mandible, followed by immediate implant-supported fixed rehabilitation. Thalassemia is a common autosomal recessive genetic disorder that can lead to osteoporosis and an increased prevalence of periodontitis. Two previously published cases of thalassemia patients undergoing implant rehabilitation have shown favorable clinical outcomes(C M Misch1998 & Zafer Ozgur Pektas 2015). This case supports the feasibility of immediate implant rehabilitation in patients with mild thalassemia. This case represents the first complete report of immediate implant rehabilitation in both jaws of a thalassemia patient. All implants demonstrated clinical success, and no significant complications were observed during follow-up, providing a clinical reference for the development of treatment protocols for similar conditions in the future.
