Radiographic Assessment Of Retromolar Limits In Mandibular Molar Distalization
This study identifies anatomical limits of mandibular molar distalization and evaluates panoramic radiography's diagnostic accuracy versus cone beam computed tomography (CBCT). 126 patients (252 samples) underwent pre-treatment panoramic radiography, lateral cephalometry, and CBCT. Retromolar space was measured at nine sites (CEJ/mid-root/apex × buccal/distal/lingual) using Invivo software. Parameters included: cortical bone distances, second molar (MM2) mesiodistal/buccolingual angulation, third molar (MM3) position, mandibular second molar to the anterior edge of the mandibular ramus (M7DRamus), and vertical growth patterns. Regression and diagnostic accuracy analyses were performed. For anatomical limits, retromolar space decreased coronally to apically, with lingual apical cortex as the primary constraint (89% of cases had Apex-L <1 mm). For Biomechanical factors, space correlated with MM2 mesiodistal (β=0.23, p=0.01) and buccolingual angulation (β=-0.16, p=0.049), MM3 position (β=0.19, p=0.036), and M7DRamusPa (β=0.57, p<0.001). Hypodivergent patients had 58% greater space than hyperdivergent (3.1±0.9 mm vs. 1.4±0.5 mm, p<0.001). For imaging accuracy, panoramic M7DRamus predicted CBCT Apex-L (R²=0.33, p<0.001) but overestimated space in hyperdivergent (+1.2 mm bias) and underestimated in hypodivergent (-0.6 mm). For diagnostic accuracy, hypodivergent PPV=0.99 is higher than hyperdivergent PPV=0.61. Lingual apical cortex critically limits distalization. While panoramic M7DRamusPa aids prediction, its accuracy declines in hyperdivergent patients. CBCT remains indispensable for complex cases, particularly those with vertical growth abnormalities.
