Same-Site Autograft For Alveolar Remodeling.

To address the clinical problem of severe horizontal bone defects in oral implantology, this study proposes an innovative surgical technique, "Guided Bone Regeneration with autogenous bone block technique in the same surgical site".This procedure was designed according to the differences in anatomical characteristics of the defect site, and three types of options were used. For severe horizontal bone defects in the anterior maxilla, the surgical procedure involves taking autogenous bone blocks from the buccal or labial side of the same surgical site and placing them horizontally into the defect at the crest of the labial alveolar ridge, combined with GBR;for severe horizontal defects in the mandible accompanied by thick cortical bone at the ridge crest, the surgical procedure involves extraction of the bone block from the crest of the alveolar ridge in the same surgical field with 180° rotation and fixation combined with GBR;for severe horizontal mandibular bone defects without crestal cortical bone, the surgical procedure involves extracting the bone block from the root apex in the same surgical field with a 180° rotation and fixing it after translating it to the labial and buccal sides of the alveolar ridge and to the crestal apex, combined with GBR. This procedure avoids the need to open a second surgical zone and shows good predictability in treating severe horizontal bone defects and accurately restoring the bone contour. This procedure provides a minimally invasive solution for the clinical management of complex bone defects.
