The Adjuncts Of Lasers In Modified Apica Lsurgery

In cases where preoperative natural root length is suboptimal, conventional 3mm apical resection may compromise fracture resistance. This study proposes a modified apical surgery involving 1mm apical resection combined with laser-assisted apex sealing and disinfection. Using extracted human teeth, we conducted three parallel experiments: (1) fracture resistance testing via universal testing machine on roots with 12mm,11mm,10mm residual length following 1mm/3mm resections to explore the indication of the modified apical surgery ; (2) scanning electron microscopic (SEM) evaluation of dentinal tubule and simulated lateral canals sealing using standardized dentin discs; (3) confocal laser scanning microscopic (CLSM) analysis of periapical biofilm eradication on resected apices after laser irradiation . It was demonstrated that when the residual length of roots was 10mm,it made differences between 1mm and 3mm resection groups (P<0.05). Laser irradiation achieved complete obturation of lateral canal orifices through dentin melting and significant biofilm reduction . During the modified apical surgery procedure in this experiment, laser is proved effective in sealing the apical region and controlling infection. These findings validate the technical feasibility of minimally invasive apical surgery with laser adjuncts.
