Angle Class II Malocclusion & Impacted Teeth: Orthodontic Management

This report presents an innovative multidisciplinary approach combining surgical-orthodontic traction with modified Twin Block therapy to simultaneously address mandibular retrognathia and multiple impactions (#23,24,25) in a growing patient. Case Description: A 13-year-old male presented with a six-month history of failed eruption in the left posterior quadrant following natural exfoliation of primary molars. Clinical examination: Facial: Convex profile with retrognathic mandible; Dentition: Mixed dentition with retained #63 and impacted (#23, #24, #25); Occlusal: Bilateral Class II molar relationship with deep overbite and increased overjet; Radiographic: bony impaction of #23, #24, #25 due to severe crowding. Treatment Protocol: 1. Impacted Tooth Traction; 2. Functional Orthopedic Therapy; 3. Fixed Appliance Therapy. Treatment Progress: A customized maxillary Nance appliance with traction hooks was fabricated. Following surgical exposure of #24,#25, controlled traction successfully aligned these teeth into the arch. Concurrently, a modified Twin Block appliance was utilized, incorporating a labial bow converted into a traction hook to simultaneously advance the mandible and facilitate eruption of #23. Subsequent fixed appliances achieved full dentition alignment and established Class I molar relationships. This complex case required staged management. Key considerations included: 1. Strategic traction vectors to upright impacted teeth while resolving arch-length discrepancies. 2. Functional appliance modification enabling dual-action therapy—mandibular advancement and impacted canine traction. 3. Sequential treatment phases prioritizing skeletal correction prior to detailed occlusal finishing. This approach highlights the importance of customized biomechanical strategies in managing concurrent skeletal and dental challenges in growing patients.

Abstract
