Digital-Assisted Multidisciplinary Approach For Anterior Teeth Aesthetic Rehabilitation

This case demonstrates a multidisciplinary strategy integrating digital planning and tissue preservation to resolve complex anterior aesthetic challenges, emphasizing innovation in balancing functional and aesthetic outcomes. A 25-year-old male presented with a dislodged crown (#21) and a gingival mass (#11). Examination revealed subgingival caries (#21), a fibrotic lesion (#11), and compromised dental proportions. CBCT confirmed inadequate root canal fillings (#11, #21) and caries (#12). Interventions: Periodontal therapy and oral hygiene education. Partial lesion excision (#11) preserving 0.5mm connective tissue. Root canal retreatment (#11, #21). Digital design: DSD analysis and 3D-printed surgical guides (Exocad DentalCAD) for crown lengthening. Provisional crowns (PMMA) and composite restoration (#12). Final zirconia crowns (Katana Zirconia) via intraoral scanning (Medit 5.0). Traditional lesion excision risks aesthetic compromise through tissue loss. This case prioritized partial resection and digital-guided crown lengthening to preserve gingival architecture, achieving predictable outcomes. Digital workflows (DSD, 3D guides) enhanced precision, aligning with evidence supporting digital aesthetics. Phased rehabilitation and patient compliance ensured stability. The protocol—minimally invasive excision, digital planning, and staged rehabilitation—provides a model for harmonizing oncological safety and aesthetics. It highlights multidisciplinary synergy, tissue preservation, and long-term maintenance as pillars of sustainable anterior rehabilitation.

Abstract

This case demonstrates a multidisciplinary strategy integrating digital planning and tissue preservation to resolve complex anterior aesthetic challenges, emphasizing innovation in balancing functional and aesthetic outcomes. A 25-year-old male presented with a dislodged crown (#21) and a gingival mass (#11). Examination revealed subgingival caries (#21), a fibrotic lesion (#11), and compromised dental proportions. CBCT confirmed inadequate root canal fillings (#11, #21) and caries (#12). Interventions: Periodontal therapy and oral hygiene education. Partial lesion excision (#11) preserving 0.5mm connective tissue. Root canal retreatment (#11, #21). Digital design: DSD analysis and 3D-printed surgical guides (Exocad DentalCAD) for crown lengthening. Provisional crowns (PMMA) and composite restoration (#12). Final zirconia crowns (Katana Zirconia) via intraoral scanning (Medit 5.0). Traditional lesion excision risks aesthetic compromise through tissue loss. This case prioritized partial resection and digital-guided crown lengthening to preserve gingival architecture, achieving predictable outcomes. Digital workflows (DSD, 3D guides) enhanced precision, aligning with evidence supporting digital aesthetics. Phased rehabilitation and patient compliance ensured stability. The protocol—minimally invasive excision, digital planning, and staged rehabilitation—provides a model for harmonizing oncological safety and aesthetics. It highlights multidisciplinary synergy, tissue preservation, and long-term maintenance as pillars of sustainable anterior rehabilitation.
