Simplify Customized Post-Crown Placement Procedure With Verification Jigs

This clinical case utilized intraoral scan data of a prepared residual root canal to fabricate a customized digital guide, which directed the precise placement of the post-and-cores and final crowns restoration. A Female patient presented with the chief complaint of "defect in upper anterior teeth for one week. "She had undergone root canal therapy on the upper anterior teeth with no residual pain. She sought restoration with crowns. Teeth 12, 11, 21and22 exhibited existing occlusal restorations. Residual root height measured 2mm. No percussion pain or mobility was observed. Root canals displayed well-condensed obturation materials and no periapical radiolucency was noted. The patient was diagnosed with teeth defect, and fiber post-cores with all-ceramic crowns restoration is planned. Post space preparation was performed, followed by intraoral scanning. Post-and-cores, crowns and positioning guides were digitally designed and milled using CAD/CAM technology. Under guidance of the positioning guide, the post-and-cores and crowns were sequentially seated and adhesively cemented. Simultaneous cementation of multiple post-and-core restorations carries inherent risks of misalignment and rotational displacement, which may hinder the seating of crowns. In this case, a positioning guide was employed to achieved passively constrained seating of the restorations, enabling synchronous, precise, and stable cementation . The positioning guide requiries a shared path of insertion for restorations to ensure predictable guide seating. While guide fabrication incurred additional laboratory costs and relied on radiographic verification, this approach significantly reduced clinical chair time and occlusal risk.

Abstract

This clinical case utilized intraoral scan data of a prepared residual root canal to fabricate a customized digital guide, which directed the precise placement of the post-and-cores and final crowns restoration. A Female patient presented with the chief complaint of "defect in upper anterior teeth for one week. "She had undergone root canal therapy on the upper anterior teeth with no residual pain.
