Multidisciplinary Collaborative Treatment Of High-Angle Class II Malocclusion

This case report details an 11-year-old male with high-angle class II malocclusion, individual anterior crossbite and high caries risk. A multidisciplinary strategy integrating biomechanics, caries risk stratification, and minimally invasive interventions aimed to optimize occlusal function and long-term oral health. Clinical findings included a convex profile, deep overbite and overjet, individual anterior crossbite and FMA (36.8°), high caries risk. Treatment plan: 1) Extraction of the first premolars; 2) Maxillary mini-screw for anterior space creation and distalization; 3) Orofacial myofunctional therapy and exercise rehabilitation therapy; 4) Post-orthodontic resin infiltration for anterior white spots and NaF remineralization for posterior regions. Key innovations:1)Vertical Control: Premolar extraction with mini-screw anchorage minimized mandibular rotation, improving the skeletal vertical pattern.2)Cavity Prevention: OHI and fluoride caries prophylaxis.3)Aesthetic-Functional Zoning: Resin infiltration restored anterior aesthetics with microhardness recovery, whereas 5%NaF promoted remineralization of white spot lesions in posterior teeth.4) Multidisciplinary Synergy: ENT collaboration improved airway function, enhancing postural stability. This case successfully addressed a high-angle skeletal class II malocclusion with individual anterior crossbite through multidisciplinary collaboration. The integration of mini-screw-driven precise force application and three-dimensional anterior control, not only improved facial convexity and occlusal function but also provided innovative insights into caries prevention during early orthodontic intervention. Concurrently, early-phase lip muscle training and cervicocranial posture rehabilitation synergistically reduced dental compensation while enhancing airway patency. This comprehensive protocol offers a valuable reference for managing complex high-angle malocclusions.

Abstract

This case report details an 11-year-old male with high-angle class II malocclusion, individual anterior crossbite and high caries risk. A multidisciplinary strategy integrating biomechanics, caries risk stratification, and minimally invasive interventions aimed to optimize occlusal function and long-term oral health. Clinical findings included a convex profile, deep overbite and overjet, individual anterior crossbite and FMA (36.8°), high caries risk. Treatment plan: 1) Extraction of the first premolars; 2) Maxillary mini-screw for anterior space creation and distalization; 3) Orofacial myofunctional therapy and exercise rehabilitation therapy; 4) Post-orthodontic resin infiltration for anterior white spots and NaF remineralization for posterior regions. Key innovations:1)Vertical Control: Premolar extraction with mini-screw anchorage minimized mandibular rotation, improving the skeletal vertical pattern.2)Cavity Prevention: OHI and fluoride caries prophylaxis.3)Aesthetic-Functional Zoning: Resin infiltration restored anterior aesthetics with microhardness recovery, whereas 5%NaF promoted remineralization of white spot lesions in posterior teeth.4) Multidisciplinary Synergy: ENT collaboration improved airway function, enhancing postural stability. This case successfully addressed a high-angle skeletal class II malocclusion with individual anterior crossbite through multidisciplinary collaboration. The integration of mini-screw-driven precise force application and three-dimensional anterior control, not only improved facial convexity and occlusal function but also provided innovative insights into caries prevention during early orthodontic intervention. Concurrently, early-phase lip muscle training and cervicocranial posture rehabilitation synergistically reduced dental compensation while enhancing airway patency. This comprehensive protocol offers a valuable reference for managing complex high-angle malocclusions.
