One Case Of Periodontal-Orthodontic Treatment For Severe Periodontitis
Severe periodontitis damages periodontal structure, function, and aesthetics. Treatment requires balancing function restoration and aesthetics. This article reports a combined periodontal and orthodontic case for reference. A 39-year-old female had 2-year upper anterior tooth space enlargement and gingival bleeding. Intraoral examination: The Plaque Index was 3; the Calculus Index was 3. Gingiva was congested, edematous, dark red and soft. The Bleeding Index was 4. Many teeth had grade II or III mobility. The probing depth was 4-12 mm, and the attachment loss was 3-12 mm. Teeth 12-22 had fan-shaped displacement, 32-42 had torsional displacement. There were 1-3 mm spaces between 13-23 and 32-42. Teeth 12-22 and 32-42 had premature contacts, with deep anterior overbite. Imaging examination: Whole mouth alveolar bone was resorbed to 1/2 to 2/3 of root length, some with vertical resorption. Diagnosis: Generalized severe chronic periodontitis. Treatment: oral hygiene education, scaling, root planing, 33-43 stabilization, occlusal adjustment, periodontal surgery, orthodontics, and supportive periodontal therapy. Basic and surgical treatment controlled inflammation and achieved bone repair. Combining orthodontic treatment is beneficial to the regeneration of periodontal and bone tissues, and an ideal treatment effect can be obtained. Appropriate orthodontic treatment not only does not aggravate periodontal inflammation but also repairs the bone defect, increases the height of the supporting bone, and corrects the malocclusion. Standardized combined periodontal and orthodontic treatment effectively controls severe periodontitis inflammation and restores function and aesthetics.
