Non-Surgical Periodontal Therapy Improve Thyroid Function: A Case Report

This case report highlights the interplay between chronic periodontitis and thyroid function in a 24-year-old female with hypothyroidism, emphasizing the potential systemic impact of periodontal health on endocrine regulation. A 24-year-old female diagnosed with hypothyroidism and chronic periodontitis had been on levothyroxine (Synthroid) for four years. Despite consistent medication, her thyroid-stimulating hormone (TSH) levels remained elevated, indicating suboptimal thyroid control. Concurrently, she presented with chronic periodontitis, characterized by gingival inflammation and periodontal pocketing. Following a comprehensive non-surgical periodontal treatment, including scaling, root planning and cleaning, her periodontal health significantly improved. Remarkably, post-treatment laboratory tests revealed a notable reduction in TSH levels, which subsequently normalized. The observed improvement in thyroid function following periodontal therapy suggests a potential link between chronic periodontitis and systemic inflammation, which may exacerbate hypothyroidism. Chronic inflammation from periodontitis could impair thyroid hormone metabolism or increase the demand for levothyroxine, thereby elevating TSH levels. The resolution of periodontal inflammation likely reduced systemic inflammatory burden, facilitating better thyroid hormone regulation. This case underscores the importance of addressing periodontal health in patients with hypothyroidism, particularly those with suboptimal thyroid control despite medication. Non-surgical periodontal therapy may serve as an adjunctive intervention to improve thyroid function, highlighting the need for interdisciplinary care in managing systemic conditions influenced by oral health.
