Treatment Of Severe Osteogenic Class III Antijaw With Multidisciplinary Combination
The application of multidisciplinary combined therapy (orthodontic-implant-repair) in severe type III osteogenic antijaw cases, evaluate the feasibility of non-bone implant implantation after local osteogenesis promoted by orthodontic tooth movement, and analyze the improvement effect of compensatory orthodontic treatment on occlusal function and facial aesthetics. This case was an adult patient with severe bone class III antimaxillary disease. Permanent dentition, 25, 46 were missing, upper and lower anterior teeth were dispersed in a large space, and the mandible could retreat but not back to the opposite edge (ANB: -7°, 1-SN: 123°, 1-MP: 90°, repeatedly covering 5mm). CBCT showed 25 bone mass deficiency, low maxillary sinus position, and buccal gum depression. After surgery, bone grafting with implants was avoided, and the coverage was from -5mm to 2mm, and the anti-jaw was lifted, and the masticatory efficiency of the patients was improved. Orthodontic tooth movement induced distraction osteogenesis has biological advantages to increase the height and width of alveolar bone and provide a better implantation site for implantation, especially for mild to moderate bone defects.;Limitations of compensatory therapy: Severe skeletal deformities (ANB<-8°) still require orthognathic surgery, and this protocol is more suitable for patients who refuse surgery and have low facial requirements. This case provides a new idea for the bone class III of critical orthognathic surgery. Compensatory treatment combined with osteogenic movement can optimize the conditions of bone implantation, reduce the need for bone grafting, and achieve functional and aesthetic repair.
