Efficient Open Bite Correction In Periodontitis Patient With Short Roots

Patients presented with hyperdivergent anterior open bite, complicated by short-rooted teeth (crown-root ratio <1:1) and convex profile, typically necessitate extraction-based orthodontic intervention for functional and aesthetic rehabilitation. Notably, teeth with roots shorter than two-thirds of normal length face a tripled risk of orthodontically induced root resorption, and this risk is critically compounded by severe periodontitis. The significance of light-force orthodontics is amplified in such high-risk scenarios. A 25-year-old female who complained of convex profile. Examination revealed skeletal Class II malocclusion (ANB 5.2°; FMA 30.2°), Angle Class II division 1, and 4-mm anterior open bite, particularly with shortened maxillary central incisor roots (5-6 mm) and generalized moderate periodontitis (PD: 2-7mm), accompanied by complications such as absence of maxillary right first premolar, severe bone loss in mobile maxillary right second premolar. By employing controlled, physiologically adaptive forces, we achieved optimized tooth movement with minimal mechanical stress on dental and periodontal structures, thereby reducing the risk of incisor roots resorption, alveolar bone loss, and tooth mobility. Ultimately, functional occlusion with harmonious facial aesthetics were attained. Biologically compatible forces with temporary anchorage devices play a pivotal role in managing cases with compromised periodontal and anatomical conditions. The case demonstrates light-force mechanics enable precise tooth movement while preserving root integrity and periodontal health. Clinically, this strategy addresses functional and aesthetic demands while prioritizing long-term stability, proving its indispensability in contemporary orthodontic care for complex patients.
