Treatment For Verrucous Leukoplakia Using PDT Combined With Tretinoin
A patient with oral verrucous leukoplakia was treated with photodynamic therapy(PDT) and tretinoin, and the lesion was effectively controlled and resolved. Medical History: The patient initially noticed white oral lesions two years ago, accompanied by discomfort when consuming spicy or irritating foods. Personal History: No history of smoking or alcohol consumption. Oral Examination: A white plaque (2.5 cm × 1 cm) was observed on the buccal gingival margins of teeth 44–47, with a uniform texture and slightly elevated surface. Histopathological Findings: Biopsy of the buccal gingiva (44–45) confirmed verrucous leukoplakia with mild to moderate epithelial dysplasia. Diagnosis: Verrucous leukoplakia. Treatment & Follow-up: PDT Parameters: Wavelength 635 nm, power 300 W, exposure time 7 min/spot, power density 160 J/cm² (3 spots per session). Regimen: Administered biweekly for a total of 4 sessions. Outcome: At the 3-month follow-up, the lesions were under control, and treatment was switched to oral tretinoin (10 mg/day). By the 4-month review, the lesions had completely resolved. Oral leukoplakia is a potentially malignant disorder, and effective management requires close monitoring and adaptable treatment strategies. This case highlights: Patient compliance is crucial in PDT. The combination of PDT and tretinoin indeed contributed to lesion regression. Timely adjustment of the treatment plan is crucial during therapy. The combination of PDT and tretinoin is effective in inducing lesion remission and controlling disease progression in this case.
