Pulpotomy Treat Traumatized Mature Permanent Incisor With Pulp Exposure

Pulpotomy is shown to high success rates in treating mature permanent teeth by correctly treatment and infection control. This study aimed to illustrate the successful management of a permanent incisor with complicated crown fractures. A 26-year-old man presented to our department with a complaint of traumatic injury to the upper front teeth after a physical assault. Intraoral examination revealed a complicated crown fracture in the left upper central incisor with fractured fragment intact and attached to the palatal gingival, the teeth showed mild pain on percussion and a slight sensitive response to the cold test. CBCT conformed that no root fractures or peri-radicular radiolucency were detected. A pulpotomy followed by reattachment was decided upon the patient. The patient informed consent to treatment. After local anesthesia, the teeth were isolated with a rubber dam. Pulpotomy and hemostasis was performed followed by placement of 2 mm layer of MTA(PROROOT, Dentsply USA) . Glass ionomer (Ketac, 3M USA) was based upon MTA. The fractured fragment was reattached using composite resin luting agent (Z350 XT,3M USA). Polishing surface and adjusting occlusion of traumatized teeth. Teeth were monitored clinically and radiographically, the esthetic and masticatory situation remained stable over a one-year period Pulpotomy many advantages over conventional RCT: i) treatment procedure is technically less challenging;ii) it preserves the pulp biological function; iii) lowering the risk of fracture The case indicates that pulpotomy should be regarded as the treatment of choice for mature teeth that have suffered complicated crown fractures.
