Application Of Digital Surgical Techniques Of Trauma-Induced Occlusal Disorder

This case tentatively applied digital surgical techniques to overcome the limitations of anatomical reduction and achieved functional occlusal reconstruction for the purpose of patient 's comfort. It provides a feasible solution for traumatic occlusal disorders caused by severe maxillofacial trauma. Information: Male, 33 years old. Open bite after trauma for 3 days. Examination: Facial symmetry is noted, with limited mouth opening and an anterior open bite. CT scan shows a right premolar sagittal fracture and a left condylar neck fracture. Diagnosis: Multiple mandibular fractures. Treatment procedure: preoperative preparation (a. Virtual occlusion planning; b. 3D anatomical model-guided reconstruction; c. 3D surgical guide fabrication.) Intraoperative navigation (d. Place the surgical guide in position; e. Guided fracture fixation.) Postoperative occlusal stabilization (f. Intermaxillary traction with rubber band). The patient’s bilateral condylar asymmetry suggests that pre-injury occlusion achieved functional balance via temporomandibular joint adaptation, not an ideal anatomical state. The reduction effect of traditional surgical methods is affected by many factors such as surgical field, swelling and bleeding in the surgical area. Our surgical goal prioritized functional occlusion over forced anatomical reduction, aligning with the ‘physiological reduction’ concept. Through virtual simulation and 3D-printed surgical guides, digital surgery can predict and design the reduction position of fracture segment in advance, achieve the purpose of restoring ideal occlusion and condylar position. Digital surgical technology offers a feasible solution for traumatic occlusal disorders through the guided of preoperative virtual planning and intraoperative guided.
