A Case Of Open Bite Treated With Extraction Orthodontics
A 32-year-old male with anterior open bite, lip protrusion, and childhood oral habits underwent orthodontic treatment for skeletal Class II high-angle malocclusion. Goals included aligning teeth, normalizing overjet/overbite, and improving facial aesthetics. Challenges: managing periodontal health, TMJ stability, and vertical control. Multidisciplinary care ensured systemic risk mitigation and long-term stability. Severe anterior open bite (5-5), bimaxillary protrusion, and narrow maxillary arch were addressed via extraction of four premolars, fixed appliances, TPA, and bite blocks. Over 2.5 years, open bite resolved, incisors retracted, and facial profile improved. Post-treatment showed normalized occlusion and balanced soft tissues. Vertical control via TPA/bite blocks prevented molar extrusion. Torque adjustments minimized root risks. Collaboration with periodontal/TMJ specialists ensured safety. Tongue training reduced relapse. Challenges: compromised molar support and anterior root proximity. Highlights individualized biomechanics in complex cases. Successful correction through extraction, vertical control, and functional therapy. Key lessons: 1.Vertical management tools (TPA/bite blocks) counter hyperdivergence. 2.Multidisciplinary monitoring ensures safety. 3.Tongue training enhances stability. Emphasizes holistic approaches for functional-aesthetic outcomes.
