Novel Strategy For Iipp In Molar Region
This strategy utilizes a surgical guide to precisely place tapered macro-threaded implants, achieving primary stability and enabling immediate loading. A 35-year-old female patient presented with a non-restorable right mandibular first molar. Clinical and radiographic evaluation revealed no acute infection and sufficient apical bone volume for primary stability. Following minimally invasive extraction, the placement of a Straumann BLX ® (Institute Straumann AG) achieved an insertion torque of 50Ncm. The jumping gap was filled with sticky bone graft and covered with a CGF membrane. Immediate postoperative digital intraoral scanning enabled the fabrication of a 3D-printed resin provisional crown, delivered the same day. After three months, the definitive prosthesis was fabricated by duplicating the provisional restoration parameters. Radiographic evaluation revealed no significant marginal bone resorption. Clinical evaluation revealed a peri-implant keratinized tissue width exceeding 3 mm, accompanied by a Jemt papilla index score of 2. To ensure the success of immediate loading, the provisional crown design was guided by electronic facebow to establish light occlusal contacts in maximum intercuspation. It also featured, anterior guidance for posterior disocclusion during protrusion and working-side canine guidance with non-working clearance in lateral excursions. Sticky bone graft demonstrates moldable morphology and ease of handling, enhancing hard tissue regeneration and maintaining marginal bone stability after immediate loading. The novel IIPP protocol in the molar region ensures primary stability, rapid occlusal recovery, and soft tissue preservation, while enhancing patient satisfaction by optimizing the treatment flow.
