Digital Implant Restoration In Edentulous Maxilla: 36-Month Follow-Up

This case demonstrates a digitally assisted, prosthetically guided approach for implant-supported fixed rehabilitation in a patient with edentulous maxilla and severe alveolar bone atrophy. Stable outcomes were maintained throughout the 36-month follow-up. A 60-year-old female requiring implant-supported fixed maxillary rehabilitation. A transitional diagnostic denture was fabricated with DSD-based aesthetic analysis using 2D facial photographs. After 3 months, the denture served as a reference for definitive prosthetics. Final impressions, intraoral scans, and CBCT data were digitally merged to design surgical guides. Six implants were placed with bilateral sinus augmentation, ensuring even distribution and parallelism. After 6 months, intraoral scans with scan abutments captured implant positions and soft tissue contours, aligning with prior occlusal/prosthetic data to transfer transitional denture parameters to an implant-supported provisional prosthesis. Following 1 month of stable function and patient satisfaction, dynamic occlusal records (JMA electronic facebow) and digital scans guided definitive prosthesis. Follow-ups at 2 weeks, 3/6/12/24/36 months confirmed stable occlusion, intact prostheses, healthy peri-implant tissues, and successful osseointegration. This case used digital technology to transfer occlusal and aesthetic information step-by-step to the final prosthesis, achieving prosthetically guided implant design. Postoperative maintenance included oral hygiene education, and occlusal adjustments. At 36 months, implants and prostheses were stable Digital technology greatly improves the accuracy of implant restoration, achieving prosthetically guided precision implantation. Good oral hygiene maintenance and regular occlusal check-ups are necessary to ensure long-term stability of implant dentures.
