Robot-Assisted Anterior Implant Placement With Intraoperative Digital Provisionalization

This case demonstrates an integrated digital workflow that combines robotic computer-assisted implant surgery (r-CAIS) with the intraoperative delivery of pre-designed digital provisional prostheses, addressing patients’ demands for same-day aesthetic restoration. A 19-year-old male patient presented to the hospital clinic with a traumatic loss of tooth #21 and a previously restored tooth #11 with a zirconia crown. Cone-beam computed tomography (CBCT) confirmed adequate bone volume for implant placement (height: 15.8 mm; width: 6.4 mm; mean HU: 380 ± 32). 1. Digital Workflow: - The implant position at site #21 was planned with a 1.5 mm safety margin using 3Shape Implant Studio®. - A screw-retained PMMA provisional crown and a 3D-printed model were fabricated prior to surgery. 2. r-CAIS Execution: - The Langyue® Robotic System was utilized to place an implant (3.3 × 12.5mm) with a maximum positional deviation of 0.2 mm and an insertion torque value (ITV) of 35 N·cm. - Immediate provisionalization was performed using a non-functional provisional crown. 1. Precision & Stability: Robotic-assisted implantation (0.2 mm deviation) surpasses freehand surgery (1.5 mm), improving positioning and stability. 2. Efficiency: This approach eliminates 2-3 extra visits, enhancing patient experience. 3. Aesthetics & Healing: Immediate provisionalization supports soft tissue healing and aesthetics. 4. Future Impact: Robotics with digital prosthetics may set a new standard, warranting further study. This workflow advances anterior implant therapy by optimizing efficiency, precision, and patient-centered outcomes, marking a shift in modern implant dentistry.
