Two-Step Immediate Implant In The Molar Region: A Rct

This prospective case series study aimed to compare the effects of a Two-step protocol (definitive prostheses were delivered through two visits) versus a conventional immediate implant protocol on implant survival rates and hard/soft tissue dimensional changes. This study enrolled 24 patients (14 males and 10 females; aged 19 to -71 years). Customized healing abutments were fabricated and placed immediately postoperatively in the test group, whereas the control group received healing abutments. Remaining peri-implant sockets were filled with Bio-oss. All patients were delivered definitive prostheses at 3-months. The test group received definitive prostheses fabricated using Intraoral scanning data obtained immediately post-operatively, completing the implant protocol with only two steps. The control group required an additional visit for definitive prosthesis digital impression taking. The implant survival rate, peri-implant bone changes, buccal keratinized tissue width and Jemt papilla index were recorded for 6 months post-operation using CBCT and IOS. At the 6-month postoperative evaluation, the implant survival rate was 100%. Peri-implant bone changes yielded 0.23± 0.28 mm. Horizontal bone remodeling in the test group was not significant at any level. Soft tissue profile was stable. The average buccal keratinized tissue width was 3.2±0.5mm and Jemt papilla index was 2.8±0.3. This study showed that the Two-step protocol achieved promising implant outcomes with limited hard and soft tissue dimensional changes while decreasing the overall treatment time.
