A Case Of Advanced Adult Second Permanent Molar Development

This paper reports a case of successful anterior placement of adult second permanent molar with invisible appliance without bracket and fragment arch. The treatment involved replacing the first permanent molar with a second permanent molar. Key characteristics of the case included Bone Type II, hypoplasia of the mandible and chin, low-angle skeletal pattern, extraction of tooth 36, stump crowns of teeth 16 and 26, extensive defect in tooth 46, locked jaws involving teeth 14 and 15, deep overbite, deep overjet, and upright anterior teeth. In the past adult orthodontic treatment, restoration and implantation are usually used to solve the loss of the first permanent molar, and orthodontic treatment is usually used in children to restore the occlusion and function of the second permanent molar. It is very difficult for adult patients with secondary low Angle bone to move molar forward, and it is also a major difficulty in invisible correction without brackets. After 43 months of treatment, the patient exhibited a more linear facial shape, a harmonized profile, and improved lip and chin contours. The second permanent molar successfully replaced the function of the first permanent molar, achieving near Class I occlusal relationships. Additionally, the third molar was effectively integrated into the correct occlusion.
