Adult Patient With Severe Periodontitis Orthodontic Outcomes Via Lightforce Mechanics
This case highlights the interdisciplinary management of a severe periodontitis adult patient with complex malocclusion, emphasizing the feasibility of orthodontic intervention following periodontal stabilization. The integration of light-force mechanics and alveolar bone preservation strategies addresses challenges in treating periodontally compromised dentition A 27-year-old female presented with advanced periodontitis exhibited anterior spacing, III°deep overbite and III° overjet, and grade II-III anterior teeth mobility. Following periodontal flap surgery, orthodontic treatment involved extractions of 14, 24, 34, and 44. Self-ligating brackets were bonded, and a light-force protocol with 0.016 inch Australian wires was applied, incorporating differential mechanics (1/4 iOZ Class II elastics and intra-arch traction). Post-treatment evaluation confirmed achievement of normal overbite and overjet, with significant improvement in facial profile. CBCT revealed no root resorption, preserved labial alveolar bone height, and absence of fenestration or dehiscence. Light-force protocols minimized stress on periodontally compromised teeth, while self-ligating brackets optimized force control. Differential traction balanced occlusal forces, preventing pathological tooth migration. Stable bone levels contrast with traditional concerns of orthodontics exacerbating periodontitis, underscoring biomechanical precision. This case validates light-force orthodontics as a safe modality for adults with advanced periodontitis. Key outcomes preserved alveolar bone, absence of iatrogenic damage emphasize interdisciplinary planning and force modulation. It redefines therapeutic boundaries for high-risk patients, prioritizing both function and tissue integrity.
