Ai-Powered Fully Digitized Lingual Selfligating With Complex Skeletal Malocclusion

Driven by the concept of precision orthodontics, the full-process digital self-ligating lingual orthodontic technology has achieved a leapfrog development from morphological invisible orthodontics to functional occlusal reconstruction through systematic integration of three-dimensional diagnosis, intelligent algorithms, and biomechanical control. We report a case of a 33-year-old woman with skeletal class II bimaxillary protrusion. Based on the Andrews six-element design system, an orthodontic treatment plan was determined through digital jaw analysis, which involved the extraction of four first premolars. Additionally, CAD/CAM topology optimization technology was utilized to construct a personalized lingual self-ligating bracket system. The outcome included successful anterior tooth retraction, posterior occlusal interdigitation, and vertical control, resulting in improved facial aesthetics. Compared to traditional lingual orthodontics, this technology relies on digital dental modeling to accurately reproduce the morphological elements of the Andrews dental arch. It synchronously regulates the inclination of the tooth axis, torque compensation, and crown protrusion in three dimensions, ensuring that the tooth movement trajectory strictly follows physiological functional standards. The breakthroughs included: ①Based on the Andrews dental arch morphological parameters, a personalized lingual self-ligating bracket system is constructed using CAD/CAM, achieving sub-millimeter adaptation of the bracket slot axis inclination/torque; ②The topologically gradient-designed lingual self-ligating brackets enable deep adaptation of the archwire deformation force to the Andrews dental arch coordination. This technological paradigm provides a solution for complex adult invisible orthodontics that combines biomechanical adaptability and occlusal stability, marking a progress of digital orthodontics.
