Retreatment Of Five Canals In Left Mandibular First Molar

This case mainly reports the process of using Dental operating microscope and ultrasonic equipment to treat five canals in the mandibular first molar The patient is a female who has been seeking medical attention for over a month due to recurrent abscesses on her left lower posterior tooth. Clinical examination revealed filling material on the 36th joint surface, pus pockets on the buccal gum side, no percussion pain, no looseness, and no deep periodontal pockets on the buccal lingual side. The overall oral hygiene condition is average. X-ray: The filling material image of the dental crown enters the pulp, low-density shadow is seen in the root tip area, incomplete filling material is seen in the root canal, and the gum tip tracing points to the near middle root tip area. Diagnosis: 36 chronic periapical periodontitis, treatment: 36 root canal re treatment+high embedding repair. After comprehensive root canal treatment and two-year follow-up, the affected tooth achieved good treatment results. Middle mesial canals are often overlooked by clinicians due to their small and concealed openings, as well as calcification, which can affect the success rate of root canal treatment. The morphology of the mandibular first molar is complex and varied, and the combined use of Dental operating microscope and ultrasonic equipment to explore the middle mesial canal during treatment can reduce the occurrence of missing root canals .
