Facially Driven Dynamic Navigation Guided Implant Surgery For Terminal Dentition
Facial scan and 3D smile design play a crucial role in guiding implant-supported full-arch rehabilitation, ensuring optimal facial esthetics and functional outcomes. This approach facilitates face rejuvenation by restoring facial asthetics, centric relation, vertical dimension, and skeletal alignment. Superimposition and 3D rendering of facial, skeletal, and dental arches enable a more comprehensive and interdisciplinary treatment evaluation and enable precise dyanamic guided surgery. A terminal dentition patient presented with bone and soft tissue deformities, super-erupted teeth, vertical dimension collapse, and reduced inter-arch clearance. Clinical examination revealed uneven anterior teeth arrangement, a gummy smile, deep overbite, loss of posterior support, and stage III periodontitis. Data acquisition included intraoral and facial photos, facial scans, intraoral scans and CBCT. Dynamic navigation technology was utilized for prosthetically driven guided bone reduction and precise implant placement. The accurate positioning of the immediate temporary prosthesis in alignment with the preoperative digital plan based on photogrammetry and relied with facial aesthetics. The integration of multiple segmented STL files and navigation software facilitated accurate implant positioning. Despite advancements in digital workflows, precise alignment of the immediate prosthesis with the preoperative plan remained a challenge. Facially guided dynamic navigation offers a model-free, full-digital workflow for implant-supported full-arch rehabilitation. This approach enhances facial esthetics, occlusal function, and treatment predictability. Further advancements in navigation technology and prosthetic integration are needed to optimize immediate prosthesis placement, improving both aesthetic and functional outcomes.
