Local Mucous Membrane Fusiform Flap Repair Refractory Oral Base Fistula
The oral base fistula seriously affects the postoperative quality of life. Elderly people with poor general condition cannot tolerate conventional skin flap repair. So, we share the local oral floor and submaxillary mucosa skin flap technique for the treatment of refractory oral floor fistula. A 63-year-old female patient had an oral base fistula for 4 months after hyomaxillocervical joint root surgery for "tongue malignancy". She expected to be repaired. A 2*2cm fistulas could be seen in the patient's left submaxillary region, which was connected to the oral cavity, with smooth margins and sputum flowing out. We designed gingival, floor of mouth, submaxillary mucosal and skin flap along the surgical scar, used mucosal alignment to repair the oral gap, and released the outer skin for fusiform surface repair. Local flap healing was satisfactory at 6 months follow-up. It is an important clinical problem to repair stomatobasal fistula with minimum cost. The patient was old, had poorly controlled diabetes mellitus, had a maximum blood glucose of 25.2 before admission, and had a low surgical budget. The size of the wound, mucosal tissue thickness, and skin elasticity should be considered during operation. The size of the local flap should cover the wound during operation, and the mucosa should be cut layer by layer during separation. For patients with oral base fistula with small wound and more underlying diseases. This technique is good at repairing wounds, helping to restore normal eating and speech function.
