Immediate Implant Placement And Restoration In Maxillary Central Incisor
Immediate implant placement combined with provisional restoration can restore aesthetics and function promptly, while optimizing long-term outcomes through guided emergence profile development via the temporary prosthesis. A 25-year-old female patient suffered from a crown-root fracture of the maxillary central incisor. The surgical guide was fabricated using preoperative digital planning. The digitally guided immediate implant was placed following tooth extraction, with simultaneous jumping gap bone grafting to preserve buccal bone plate thickness. To address the patient’s high aesthetic demands, an immediate provisional crown was delivered postoperatively. Over 6 months of follow-up, optimal emergence profile shaping was achieved, followed by definitive restoration. At 6 months, peri-implant gingival morphology, occlusal stability, and prosthetic position remained favorable. Provisional restoration crown contouring is critical in immediate implant-provisionalization, influencing peri-implant tissue stability and long-term aesthetics. Dynamic adjustments are required based on individual anatomic factors (e.g., gingival biotype, edentulous site dimensions). However, CBCT-revealed buccal bone deficiency raises questions about the indications for jumping gap grafting, particularly in the esthetic zone, where evidence remains limited. This case highlights that digitally guided immediate implant placement with provisional restoration can achieve predictable red-white aesthetics within a short-term follow-up period.
