Soft Sinus Lift With Prf-Collagen Composite In Atrophic Maxilla

This case demonstrates a minimally invasive solution combining soft sinus elevation with PRF-wrapped collagen blocks to achieve bone augmentation in a patient with ≤2mm residual bone height, avoiding traditional lateral window techniques. A 56-year-old male patient had a three-year history of missing left maxillary posterior teeth. Following a CBCT evaluation, the bone height was less than 2 mm, and the presence of significant blood vessels in the lateral wall of the left maxillary sinus precluded the possibility of performing an external maxillary sinus lift for bone grafting. Transalveolar soft maxillary sinus floor elevation technique involves the application of a PRF membrane wrapped around the surface of the tool, followed by gentle tapping to achieve the targeted elevation. The resultant space is then filled with a PRF membrane wrapped around a small collagen block. The results four months after the operation showed that the implant had osseointegrated with the alveolar bone, resulting in a perfect result both functionally and aesthetically. This protocol has been shown to reduce the risk of mucosal perforation of the maxillary sinus. Furthermore, it avoids the lateral wall vessels and reduces the risk of massive bleeding. Moreover, it has been demonstrated to significantly minimise the incidence of postoperative complications and to promote osteogenesis. This case demonstrates the use of Transalveolar soft maxillary sinus floor elevation combined with PRF encapsulated collagen block for severe maxillary defects with success.
