Modified Spring Appliance For Flared Maxillary Incisors With Diastema
To present a modified spring appliance for correcting flared maxillary incisors with diastema, achieving rapid alignment, aesthetic preservation, and enhanced patient comfort. Ms. He, a 20-year-old female nurse, sought orthodontic intervention for a 3-year history of diastema between maxillary central incisors. Intraoral examination: Labial flaring of #11/21 with 1 mm diastema; coincident midlines; Class I overjet; reduced overbite; crossbite on #25/35; right mesial molar relationship; left neutral molar relationship. Facial examination: Proportionate vertical facial thirds; symmetrical bilateral contours; coincident menton midline. Diagnosis: Angle Class III subdivision;skeletal Class I. A modified W-shaped spring appliance (0.5mm stainless steel) was designed for localized treatment due to financial constraints and aesthetic demands, comprising a removable midpalatal plate with embedded spring arms engaging bonded palatal tubes. Full-time wear (24 h/day) and weekly mesiolabial activation (0.4–0.6 N) achieved complete correction within 3 weeks. Post-treatment stabilization involved lingual fixed retention and thermoformed appliances, demonstrating stable outcomes at 6-month follow-up without relapse. The localized treatment was adopted due to financial constraints and aesthetic priorities. Palatal tube engagement prevented self-removal to avoid force reduction. Debris accumulation necessitated her check-up once a week for hygienic maintenance, effectively preventing mucosal edema. The modified spring appliance provides rapid, effective correction of flared maxillary incisors with diastema while maintaining aesthetic integrity and patient comfort.
