Esthetics Restoration Of Fractured Anterior Teeth With Modified Cast Stent.
The fracture of anterior teeth in the maxillary is usually complicated crown-root fracture, and the common treatment include crown lengthening or direct extraction, which is aesthetically unsatisfactory. In this case, a modified cast stent was used to perform root traction on a patient whose fracture line was located to 4mm subgingival. The post- and- core restorations was repaired after gingival plasty, and finally excellent aesthetic results were obtained. The patient was a 36-year-old male, whose left maxillary cusp was fractured in an accident six months ago, with the palatal fracture extending approximately 4 mm subgingivally. 2 weeks after perfect root canal therapy, a modified cast stent was used to pull the fractured root along the long axis of the tooth. The root was maintained for two months before an all-ceramic crown restoration was performed, which achieved satisfactory aesthetic results. In the treatment of this complicated crown-root fracture, whether the tooth should be preserved or removed is controversial among experts and scholars. How to preserve the affected tooth as much as possible and improve the aesthetics and function to the greatest extent is a problem worthy of serious consideration by clinicians. The modified cast stent provides stable orthodontic anchorage, controllable traction direction, and is simple and efficient, which not only preserves natural teeth but also maintains good biological width, maximizing the aesthetic and functional improvement of the anterior tooth. Attempting to provide an effective way for the treatment of complicated crown-root fractures in anterior teeth.
